Biomechanical Comparison of Medial Patellofemoral Ligament Reconstuction and Distal Realignment for Patellar Instability

چکیده
In recent years, the medial patellofemoral ligament (MPFL) has been considered as the primary restraint of the medial patellar stabilizers [1]. Clinical reports have revealed that MPFL injury occurs in almost all patients with traumatic patellar dislocation, and attention has been focused on the repair and reconstruction of the MPFL in recurrent patellar dislocation and patellar subluxation [2]. On the other hand, realignment of the attachment of patellar tendon (distal realignment) has been performed as another surgical treatment for patellar instability. However, application of these surgical procedures for patellar instability is still controversial. The objective of the study was to compare the biomechanical effect of two major procedures (MPFL reconstruction and distal realignment) for patellar instability.

medialis obliquus, is the main passive stabilizer of the patellofemoral joint. Recently, it has been shown that the medial patellofemoral complex is the main pathoanatomy after patellar dislocation [1-3] . In addition, biomechanical studies have demonstrated that the MPFL is the main restraint against lateral patellar displacement and that repair or reconstruction of the MPFL restores normal pa...

To the Editor: I read with interest the paper by Sillanpàà et al. [1] entitled ‘‘Ligament Reconstruction Versus Distal Realignment for Patellar Dislocation’’ published in the June 2008 issue. I congratulate the authors on their study, for the longterm followup and for the comparative study with distal realignment. However, I ask for clarification of two issues. First, what are the indications f...
متن کاملMedial patellofemoral ligament reconstruction for patellar dislocation due to rupture of the medial structures after total knee arthroplasty: a case report and review of the literature.
Patellar dislocation is a well-recognized major complication after total knee arthroplasty (TKA). Treatment of this injury is determined according to the cause of the dislocation. In particular, proximal realignment, distal realignment, and lateral retinaculum release are options if patellar instability is not caused by prosthetic malposition. Here we report a case of patellar dislocation follo...

UNLABELLED Recently, medial patellofemoral ligament reconstruction has been emphasized for the treatment of patellar dislocation. This study compared the results of medial patellofemoral ligament reconstruction by adductor magnus tenodesis with distal patellar realignment in patients with recurrent patellar dislocation. Additionally, the development of patellofemoral osteoarthrosis was compared...

Recurrent patellar instability can result from osseous abnormalities, such as patella alta, a distance of >20 mm between the tibial tubercle and the trochlear groove, and trochlear dysplasia, or it can result from soft-tissue abnormalities, such as a torn medial patellofemoral ligament or a weakened vastus medialis obliquus. Nonoperative treatment includes physical therapy, focusing on strength...
